STOP! Are you protecting your student's privacy?

Student information and academic content should be contained within a password-protected environment or controlled by teacher invitation, and not discoverable by search engines or publicly viewable on the internet.

 

If the app requires the use of student PII (Personal Identifiable Information) such as name, e-mail address, or student identifier, parental approval may be required.

If you have a student who has opted out of Supplemental Digital Resources and needs to opt back in, this is the form to do so.

This is a suggested permission slip/form you can send home with students with whom you plan to use Web Apps. You can save your own copy and customize it.
